The development of Primary Care Networks (PCN) in 2019, as part of the NHS Long Term Plan, allowed General Practice to bring in a significant number of additional patient-facing roles, helping to ensure patients see the right professional at the right time and without going through an external referral process.
Our five Northumberland GP practices came together into a single PCN in April 2022. Since then, we have expanded the range of specialist roles and skills within these practices, along with general clinical knowledge and skills that add capacity to practice GP and nursing teams. This increases choice for patients, who can be seen quicker and for longer and allows GPs to focus on people with complex needs. Our Primary Care Network is strengthened by the existing NPC infrastructure and cross-practice links, as well as our close working relationship with other local partners in the health and social care system.
But that is not all; the advent of additional roles into general practice creates real opportunities for us to personalise the care we offer so that we can focus on what matters to the individual patient. We can also focus on greater provision of proactive and coordinated care, integration with local services and reducing health inequalities, particularly for those disproportionately affected by Covid-19.
Each practice in our PCN has access to:
- Living Well Coordinators
- Care Coordinators
- Mental Health Coaches and Practitioners
- Home visiting service
